To ensure that our clients receive a well-rounded care approach for their health needs, we conduct home visits and calls. Our nurse practitioners perform assessments to develop the right care plan for each client, follow up on their health progress during the plan’s implementation, and make changes at any stage during the care process when necessary. We want all our clients to achieve their desired personal and health outcomes.
